Ephese Haitian Seventh-day Adventist Church (also known as Ephese SDA Church) runs this website at ephesesdari.org. This page explains what information the website collects, where it goes, and who can see it.

Information you choose to send us

The website collects personal information only when you fill out and send a form.

  • Contact form: your name, email address, and message.
  • Pastoral care request forms: depending on the form, your name, email address, phone number, mailing address, preferred contact method, best time to reach you, preferred dates or times, and the details of your request. Every request needs an email address or phone number so the church can reply.
  • Prayer requests: your request and a way to reach you. You may leave your name off. You also choose who may see the request: the pastoral team only, or the pastoral team and the church prayer ministry.
  • Some forms ask about other people, such as a family member you are requesting a visit for, a child being dedicated or baptized, a partner for a wedding, or a loved one who has died. Some requests may include sensitive information, such as health, counseling, or family matters. Please share information about another person only if you have their permission.

Text messages

This website does not offer text message (SMS) sign-up and does not send text messages. Choosing “Text message” as your preferred contact method on a form tells the church how you would like to be contacted about that request. It does not sign you up for text messages.

Information collected automatically

  • Form protection: when you send a form, the website turns your internet (IP) address into a coded value (a hash) and uses it to count submissions, so the forms cannot be flooded. Your IP address itself is not saved with your request.
  • Hosting: the website is hosted by Vercel, which receives standard technical information, such as your IP address and browser type, when it delivers and protects the site. Vercel handles this information under its own privacy policy.
  • Maps and videos: some pages show a Google map or YouTube videos. Videos use YouTube’s privacy-enhanced mode (youtube-nocookie.com). When these load, Google or YouTube may receive information from your browser under their own privacy policies.
  • The website does not use analytics tools, advertising trackers, or cookies of its own.

How the church uses your information

The church uses what you send to respond to your message or request — for example, to contact you, schedule a visit or service, pray for you, or follow up with pastoral care.

Where it is stored and who can see it

Form submissions are saved in the church’s record-keeping system, which is hosted by Supabase. Church staff whose role allows it are notified of each new request and can open it.

Pastoral care requests are marked confidential, except prayer requests you choose to share with the church prayer ministry. In the church’s system, confidential requests can be opened only by the pastor, an associate pastor the request is assigned to, and system administrators.

Contact form messages, and prayer requests you share with the prayer ministry, are not marked confidential. They can also be opened by church leaders, such as elders and administrators, whose role gives them access.

Other than the website host (Vercel) and the record-keeping service (Supabase), the website does not send your information to any other company.

The church does not sell your information or share it with anyone outside the church, other than the service providers named in this policy that help run the website and the church’s records.

How long we keep it

The church keeps submissions for as long as they are needed for pastoral care and church ministry. There is no fixed schedule for deleting them.

Your choices

You can ask the church to show you, correct, or delete information you sent through the website. To make a request, call (401) 941-0998 or email hello@ephesesdari.org.

Children

The website’s forms are meant to be filled out by adults. Children under 13 should not send information through the website themselves; a parent or guardian can contact the church for them. Some forms, such as child dedication and baptism requests, ask a parent or guardian for information about a child.

Changes to this policy

If this policy changes, we will update it on this page and change the date at the top.
